Only Alaska and Louisiana Lag FMCSA Electronic Medical Card System

Alaska and Louisiana are now the only states that have not implemented FMCSA's electronic medical certification process for commercial drivers. Commercial Carrier Journal reported the two-state status on August 27, and FMCSA's official National Registry II Learning Center identifies the same jurisdictions as not yet implemented. The change marks major progress toward nationwide electronic handling of DOT medical examination results.

Under Medical Examiner's Certification Integration, commonly called NRII, certified medical examiners submit examination results to FMCSA's National Registry. FMCSA then transmits the information to state driver licensing agencies so medical-certification status can be added to a driver's CDLIS record. The system is designed to reduce paper handling, improve record accuracy and give states and motor carriers a more consistent way to confirm that a CDL holder remains medically qualified.

Drivers should not assume that electronic transmission eliminates every follow-up step. A CDL holder remains responsible for completing the examination on time, giving the examiner accurate license information and checking the procedure required by the licensing state. Drivers licensed in Alaska or Louisiana should continue following their state agency's instructions for submitting proof. Every driver should keep the medical examiner's certificate provided after the exam and confirm that the updated status appears correctly before the prior certification expires.

Motor carriers should continue checking the CDL motor vehicle record and maintaining required driver-qualification documentation instead of relying only on a paper card. The implementation update changes how information moves; it does not relax FMCSA physical-qualification standards or extend an expiring certificate. Prompt verification after each examination can prevent an administrative delay from interrupting a qualified driver's ability to work.
